The success of the recording depends on your cooperation. You are expected to keep track of your activities and symptoms in the diary.

Please record:

- Exercise

Any or all of the following symptoms:

- Chest Pain
- Shortness of breath
- Palpitations
- Light Headedness

Important Points to Remember:

- Do not shower, bathe or swim. Any moisture will loosen the electrodes and water will destroy the computer.
- Do not use body powder or talcum.
- Avoid using electric blankets, as they may interfere with the recording.
- Keep recorder straps around your waist or shoulder while moving about.
- Do not remove any tape that has been placed over the lead wires.
- If a lead wire comes unsnapped from an electrode, snap it back on and enter this in your diary.

NOTE: DO NOT open the monitor. DO NOT remove the monitor.

Please return the entire carrying case (straps, belt). You are fully responsible for the recording device.
